Health History and Physical Forms are required for all Kindergarten and Preschool students every year and new students entering Holy Trinity School for the first time. Health History and Physical forms must have a physician's signature.

ABOUT US
Holy Trinity School, established in 1845, is a catholic school steeped in the traditions of faith, service and academic excellence. We continue to thrive with more than 550 students attending preschool through 8th grade.
